Information garnered from this assessment is given to our Case Management team, who uses it in partnership with the resident to set short and long-term goals for obtaining self-sufficiency. Residents typically report that once they are medically and mentally stabilized, goals of independence are less overwhelming, and much more attainable.

"I came to HEP after suffering with a drug addiction for more than 20 years, I’m proud to say that I’ve been clean and sober for nearly four years. When I needed a foundation, when I needed quality people to support me, HEP was there. They treat me like an individual and not a number. I was lucky enough to have completed HEP’s “Go Healthy” chronic disease self-management program, which teaches residents effective methods for coping with medical and mental health ailments. So, when I suffered a heart attack after being in the program the training I learned in “Go Healthy” really helped me recognize the severity of the situation and seek treatment. I truly owe my life to HEP. I’m so grateful for all the assistance HEP has given me. With their help, I can see a bright future ahead.”
