Information garnered from this assessment is given to our Case Management team, who uses it in partnership with the resident to set short and long-term goals for obtaining self-sufficiency. Residents typically report that once they are medically and mentally stabilized, goals of independence are less overwhelming, and much more attainable.
Referred to Dental & Wellness
Upon intake: Clients are referred to HEP Dental & Wellness Clinic for a checkup and/or to receive treatment.
Assessed Mentally & Physically
An on-site medical and mental health assessment is performed through partnerships with Morton Plant Mease Outreach and Baycare Health Systems through the HEP Dental & Wellness Clinic.

When I needed a foundation, when I needed quality people to support me, HEP was there. They treat me like an individual and not a number. I was lucky enough to have completed HEP’s “Go Healthy” chronic disease self-management program, which teaches residents effective methods for coping with medical and mental health ailments.
