How WE Help

Information garnered from this assessment is given to our Case Management team, who uses it in partnership with the resident to set short and long-term goals for obtaining self-sufficiency. Residents typically report that once they are medically and mentally stabilized, goals of independence are less overwhelming, and much more attainable.

12-24 Hours

Referred to Dental Clinic

Upon intake: Clients are referred to HEP Dental Clinic for a checkup and/or to receive treatment.

24-72 Hours

Assessed Mentally & Physically

An on-site medical and mental health assessment is performed through partnerships with Morton Plant Mease Outreach and Baycare Health Systems.


A Positive Change

“Before finding HEP, I was bouncing around from place to place, and struggled with negative family relationships. The stress of moving frequently, and fighting with my family was really weighing me down. When I came to HEP, I not only found a stable place to call home – I found the support I needed to make life changes for the better. A major part of the help I received was through HEP’s VIP Program, a therapy service in which HEP residents can talk about our struggles and problems, and learn about different ways to cope with anxiety. The guidance I received from VIP made me feel like a weight was lifted off of my shoulders. I was really down and out when I first came here. Now, I am working with my Case Manager to look for an apartment. I can’t wait to decorate my own space, and host a house-warming party for the new friends I’ve made here!”
